Dr. Samples reviews, organizes, and summarizes your records into a clean, professional report you can hand directly to your VSO, attorney, medical examiner, or the court.
This is a clinical record review service. Dr. Samples does not file claims, represent veterans before the VA, or act as an accredited claims agent.

A Provider Who Understands the Mission

Dr. Rose Lee Samples, DNP, FNP-C, is a board-certified Family Nurse Practitioner with over 20 years of nursing experience — including service as a Major in the United States Air Force and work as a VA Medical Examiner. Her husband is currently active-duty United States Air Force.
She brings firsthand knowledge of military healthcare, occupations, records systems, and the documentation veterans need to move their cases forward.

Medical record review — up to 2,500 pages $299

Each additional 1,000 pages $100
Once your record is reviewed Dr. Samples will provide you with a full report of what you requested be researched in your records. Please see example of what a report looks like. The report will assist your VA rep, attorney and medical examiner with easy to read document.
